Application

To visit IMSTA FESTA, please fill out the application form and make a reservation. You can also apply for the seminars using the same form.

NOTE:
* If you are visiting with more then one person, please fill out one application form for each person.
* The application form is in Japanese. When filling out the form, please see the English guide below as needed.

[Application Form]

1. Fill in your name in the 1st and 2nd column (2 times), and your email address in the 3rd column.
2. Put checks on the seminars you want to attend on the seminar schedule.
* If you are visiting the IMSTA FESTA exhibition section only and not attending the seminars, please go to step 3 with no check marks on the seminar schedule.
* The seminars that have no check boxes are seminars that are fully booked.
3. Fill in the questionnaire ( this is not a required item, it asks about your gender, age, your music environment etc.).
4. Click on the "次のステップへ進む(next step)" button. A confirmation window appears.
5. Click on "この内容で送信→" to send the form and make a reservation （If you need to go back to the previous window, click on "←入力画面に戻る"）.
6. You will receive a confirmation email.
